Medical Marijuana Collective Applicants - Background Clearance Process
Section B.14 of the Los Angeles Municipal Code states, in part:
"A person who has been convicted currently or within the previous 10 years of a
felony or a crime of moral turpitude, or who is currently on parole or probation for
the sale or distribution of a controlled substance, as determined by the Police
Department, shall not be engaged directly or indirectly in the management of the
In order to demonstrate compliance with this section, any and all persons filing a Notice of Intent
to Register as a medical marijuana collective are required to submit a Live Scan report and be
cleared by the Los Angeles Police Department (LAPD) prior to such filing. In order to ensure
that the report submission process and the Police Department review can be completed prior to
submission of the application to the City Clerk, the following steps should be followed:
1. No less than six (6) weeks prior to filing with the City Clerk, choose a Live Scan provider and
submit to the Live Scan protocol. Please use the form provided by the City, as it includes
delivery information necessary to ensure the report gets to LAPD. A list of providers can be

2. After submitting the Request for Service and completing the protocol and after the Live Scan
provider has completed their portion of the form, the applicant should make three copies of the
Request for Service form. One should be kept for the applicant's records. One copy should be
mailed to LAPD at P.O. Box 30158 Los Angeles, CA 90030 with payment totaling $151. Please
make payment payable to the Los Angeles Police Department and note the name of the
collective on the payment. After the form and payment is mailed, contact Lt. John King of LAPD
at (213) 972-2566. Provide Lt. King with the DSSOLFDQW¶VQDPHFROOHFWLYHQDPHDQGDGGress of
the collective.
3. Once the Live Scan report and the payment have been received, LAPD staff will review the
report and make a determination of eligibility. Notification of this determination will be mailed to
you at the address listed on your Live Scan Request Form.
4. When the filing period opens and you file with the City Clerk, both the third copy of the Live
Scan Request Form and a copy of the eligibility determination from LAPD should be included
with your application.
Each individual to be named on the Notice of Intent to File submitted to the City Clerk
must submit a Live Scan and pay the $151 fee to LAPD. If all individuals named are not
cleared by LAPD, the collective's application will not be accepted.
