It is mandatory for all shop floor employees to adhere to the Shopfloor
Mandatory Medical Examination Requirement Procedure
Safety glasses must be worn at all
times in work areas.

Long and loose hair must be contained.

Safety shoes must be worn at all
times in the shop floors

Protective clothing must be worn.

Rings and jewelry must not be worn.

Hearing protection must be worn in
areas with hearing protection signage

PURPOSE
This aim of this procedure is to establish a medical surveillance program to safeguard
employee health through mandatory medical examination health screening at specific
interval as set forth in the WSH (Medical Examination) Regulations and as West deem
appropriate for its employees

2.0

SCOPE
Mandatory shop floor medical examination health screening includes


Full chest Xray for silica exposure



Trichloroacetic acid Urine Test / Liver Function Test for Trichloroethylene exposure



Audiometric for high noise level exposure



Tetanus for exposure to metal rust injury

Employee who may be exposed to silica, Trichloroethylene,rust and high noise
level directly due to their job function and this include:•

Employee exempted from the mandatory medical examination health screening
Typically all office employees are exempted from this requirement
3.0

PERSONS TO BE MEDICALLY CERTIFIED FIT FOR EMPLOYMENT
New employee shall undergo a pre placement medical examination by a designated
workplace doctor and be certified fit to work in such occupation within 30 days before the
date he is to commence his employment in such occupation

MEDICAL HEALTH SCREENING BUSINESS CONTRACT AGREEMENT
The HSE department will source for suitable medical health screening service provider
based on pricing and upon approval by the purchasing department, business contract
agreement will be established with details on the required medical health screening
services to be undertaken , the cost associated with each health screening and the
required timeline for the completion of the medical health screening by west employees

7.0

HEALTH SCREENING REQUEST FORM
Upon shortlisting the medical examination screening service provider, the service provider
will provide West with a copy of the health screening request form which shall be signed by
the HSE department. The form shall be hand carry by west employee who will personally
call at the clinic for their medical examination health screening and be presented during the
registration

8.0

REPORT OF EXAMINATIONS
Every worker examined should have their individual Report of Examinations as this serves
as their personal case notes. The workers’ individual forms should be completed after each
round of examinations and whenever repeat tests and other actions e.g. workplace
inspection, assessing and reducing risks, referrals, etc are carried out. It gives a
chronological record of the medical examination results and interventions, enabling you to
detect adverse trends.
Clinical examination findings and test results should be included. One form should be used
per worker per hazard. If a worker is exposed to more than one hazard, the worker’s
personal particulars need only be completed once.
