REPAIR REQUEST FORM // RA#
Please complete the repair request form in its entirety. If you are uncertain of the
mic model or serial number please give DPA’s Service Department a call.
Upon receiving the completed form, DPA will reply with an RA# along with shipping
When the mic(s) are received you will be notified via email with a diagnosis within
three to five business days. You will be emailed a detailed quote regarding repair
options/costs for your approval before any repair work is begun.
