LEVEL 1 & 2
Hello, this _________ at __________ School. Your
son/daughter has been injured and the nurse is right now
evaluating the extent of the injury. I don’t have any other
details. As a precaution, we have called the EMT’s and will be
transporting your child immediately to LRGH.
We’d like you to come to the local hospital and meet there.
How far away are you?
If you are more than 15 minutes away: please call hospital
admittance# ________ to verbally authorize treatment
immediately as the ambulance arrives.
You can best help right now by driving to the hospital and
registering your child in the ER.
Do you have a cell phone number so I can give you updates as
they are available?
Is there anyone you’d like to me to call?
