T-Shirt Order Form 2016-17

Dear Parents,
All band, chorus, and strings students have the opportunity to purchase a music T-shirt this year
for $7.25. Students will wear their T-shirts for all music performances. Purchasing the T-shirt is
completely OPTIONAL – if you choose not to buy it, your student may wear a plain black
shirt/blouse for performances. If your student already owns a Willow Springs music T-shirt, they
can use the same one for this year.
We will be having a T-shirt design contest to feature a new illustration this year! If your student
is interested in submitting a design, please see the back of this form for more information. (This
new T-shirt will still be black.)
If you are interested in ordering a T-shirt, please fill out this form and return it with $7.25
attached. There are two ways to submit your payment:
1. Check – made payable to Willow Springs Elementary School. Write your student’s name
on the check.
2. Cash – EXACT CHANGE ONLY. Place in an envelope or Ziploc bag with your student’s
name written on it.

We need your help designing the new logo for the Willow Springs music T-shirt! Draw your design in the
blank box below. Here are some guidelines for your illustration:
1. Your picture should be music-themed. It should include all 3 groups – band, chorus, and strings.
2. Your picture must include the following words: “Willow Springs Music,” “Band,” “Chorus,” and
“Strings.”
