for details. Last date for receipt of application (Self attested photocopies of all Educational (10, 10+2, Dip, UG,
PG)/Experience certificates along with two photos to be submitted with application form) is 25 Mar 2017. All
Medical and Para Medical candidates should be registered with their respective national/state level councils. Self
attested photocopy of registration certificate should be submitted along with the application form. Incomplete
application in any form will be rejected forthwith. Application be addressed to “Station Headquarters(ECHS),
Fort Saint George, Chennai – 600 009” Interview will be conducted on 05 Apr 2017 (Wednesday) from 0930
hrs at Station Headquarters (ECHS), Fort Saint George, Chennai -09. Candidate will bring original certificate,
photocopy of testimonials/experience certificate, as applicable and one passport size photograph at the time of
interview. Individual placed on the reserve list can be appointed in any other polyclinic if the vacancy arises
subject to his willingness.
