No dental insurance?

Q​. ​How do I sign up?
A​. New patients​ ​all you have to do is schedule
a new patient exam and x-rays then let the front
office know that you would like to sign up for
the plan.
Current patient​ that have been in within
the last 6 months you just need to call the office
to sign up for the plan.
Q.​ ​When does my membership start and end?
A.​ Your membership covers you for 12
consecutive months. Starting the day your
payment is received.

membership plan has been designed
specifically for patients without dental
insurance who are looking for affordable
dental care. Our membership plans are
focused on long-term prevention.
This is an annual membership plan that
allows individuals and families to receive
quality dental care at 20% off of our
standard fees.

Q.​ ​Can I split up the payment?
A.​ Yes you can split it up. There is an
administration fee if membership is split up into
different payment options. (Please see
membership dues.)
​What else should I know?

Membership plans are ​NOT​ dental
insurance plans.
The membership cannot be used with
dental insurance or other discount
The membership is non transferable
No credit or refunds will be given even
if you choose to not be seen in the
covered time frame.

What is included in all plans?
Membership Types

2 examinations
2 cleanings (Prophylaxis)
2 Fluoride treatments
All needed digital x-rays
2 emergency exams per year
2 Oral health screenings
20% discount on all other

*Our periodontal membership plans include
all of the services listed above with up to 2
additional cleanings per year.
* Membership level necessary to maintain
dental health is determined by the Dentist at
the initial examination and is subject to
change yearly if your dental needs change.
