Download the app on your smart phone.

STEP 1

Click on “First Time Here” in the bottom
right and enter the primary member’s
information:
> Last Name
> D.O.B.
> Zip Code
> Email address
> Password
> Phone number

STEP 2

Follow the prompts and create
your account. Feel free to skip
the ‘insurance syncing’ step.
> Accept the Terms & Conditions
and then continue into the app.
> Ensure your phone number
is accurate as this is the
number the physician network
will call you back on when you
click CALL DOCTOR.

STEP 3

Add the physician access number
866.703.1259 to your phone.
> Your membership is verified
using the primary’s last name,
D.O.B. and zip code. So, you’ll
never need a group or member
number to access service.
