3.5 毫克/:减少 1 片/餐 盐酸司维拉姆说明:由于 Renagel 无全身性吸收,所以安全性高,可以有效控制血磷值并 且不会导致高血钙症等副作用。磷能解锭的使用禁忌,主要是对低血磷、大肠阻塞、以及 对该药成份会过敏的患者。 长期洗肾的病友,还需考虑软组织钙化,尤其是动脉钙化问题,这可能与心脏血管疾病有 密切关联。Braun 等人在二年的长期临床试验中,研究了 114 位洗肾病友,发现 Renagel 与 其它含钙制剂的降磷效果相当。但使用含钙制剂的病人,有明显较多的高钙血症与 PTH 的 过度抑制现象。其中高钙血症 (Ca >2.8 mmol/L) 在含钙制剂组与 Renagel 治疗组的出现比 例,分别是 19%与 0%。另一方面,使用含钙制剂的病人,其心血管钙化程度有明显的增加 (median +34% in coronary artery), 而服用 Renagel 的肾友则未观察到这个问题。 盐酸司维拉姆不良反应:诺维乐不良反应与其他唑类药物相似,最常见的治疗相关性严重 不良反应有胆红素血症、转氨酶升高、肝细胞损害以及恶心和呕吐。 盐酸司维拉姆禁忌: 对本品任何成分过敏者禁用; 禁用于低磷血症患者; 禁用于肠梗阻患者。 盐酸司维拉姆注意事项: 在患有以下疾病的患者中尚未确定本品的安全性和有效性: 1)吞咽困难 2)吞咽障碍 3)重度胃肠功能紊乱,包括未经治疗的或严重的胃轻瘫、胃内容物滞留,或者肠道运动异 常或不规律 4)活动性炎症性肠病 5)胃肠道大手术 因此,在上述患者中应慎用本品。 1)肠梗阻和肠阻塞/不完全肠阻塞 使用盐酸司维拉姆治疗的过程中,在罕见的病例中观察到肠梗阻和肠闭塞/不完全肠闭塞。 盐酸司维拉姆与碳酸司维拉姆的活性成份相同。初期的症状可能是便秘。 使用本品进行治 疗时,便秘患者应密切监测。 在发生重度便秘或其他胃肠道症状的患者中,应重新评估是 否采用本品进行治疗。 2)脂溶性维生素 根据饮食摄入和患者所患疾病的严重性,慢性肾脏病(CKD)患者体内的维生素 A、D、E 和 K 水平可能较低。 不能排除本品可能结合摄入食物中含有的脂溶性维生素。 在不摄入 补充维生素但服用本品的患者中,应定期监测血清维生素 A、D、E 和 K 的水平。推荐在必 要时服用维生素补充剂。在进行腹膜透析的患者中,建议监测脂溶性维生素和叶酸,因为 在一项临床试验的腹膜透析患者中,未测定维生素 A、D、E 和 K 的水平。 3)叶酸缺乏 目前的数据尚不充分,不能排除本品长期治疗期间有发生叶酸缺乏的可能性。 4)低钙血症/高钙血症 慢性肾脏病(CKD)患者可能会发生低钙血症或高钙血症。本品不含任何钙成份。因此, 应定期监测血清钙水平,必要时补充钙元素。 5)代谢性酸中毒 慢性肾脏病患者有可能发生代谢性酸中毒。因此应对血清碳酸氢盐和氯水平进行监测。 6)腹膜炎 进行透析的患者有发生透析相关的感染风险。腹膜炎是进行腹膜透析患者的一种已知并发 症,在一项以盐酸司维拉姆进行的临床试验中,与对照组比较,司维拉姆组中报告的腹膜 炎病例数大大增加。进行腹膜透析的患者应进行严密监测,以确保无菌技术的正确应用, 对腹膜炎相关的任何体征和症状应进行快速确认和管理。 7)吞咽困难和窒息 曾有服用碳酸司维拉姆片出现吞咽困难或食道药片滞留的罕见报道,个别患者需要住院或 干预治疗。大多数这些患者都伴有吞咽困难或食道异常的症状。对于吞咽困难的患者,服 用碳酸司维拉姆时应慎重。 8)抗心律失常和抗癫痫药品 向同时服用抗心律失常药物和抗癫痫药物的患者开具本品处方时,应特别谨慎(见(药物相 互作用))。 9)甲状腺机能减退 对于同时服用碳酸司维拉姆和左旋甲状腺素的甲状腺功能减退患者,应密切监测甲状腺刺 激激素(TSH)的水平和甲状腺功能减退的征象。 (见(药物相互作用))。 10)长期慢性治疗 在一项为期一年的临床试验中,没有发现本品蓄积的任何证据。然而,并不能完全排除长 期慢性治疗()1 年)过程中发生本品吸收和蓄积的可能性(见(药代动力学))。 11)甲状旁腺功能亢进 本品不适用于控制甲状旁腺功能亢进。在继发性甲状旁腺功能亢进的患者中,本品应在多 种治疗途径应用的前提下使用,包括钙补充剂,1,25-二羟基维生素 D3,或者其类似物的 一种,以降低全段甲状旁腺激素(iPTH)的水平。 印度抗癌药品更多详情咨询 电话:0091-9205113743 电子邮箱:lifesmarthealthcare@gmail.com 官网:www.apolopharmacy.com
