Please complete this form and then scan it into your computer and email it to
momentsvideos@hotmail.com . If you are unable to scan in the form then you can email us
the photo names, sizes and quantities without sending the form. After this we will send you
an invoice for payment.
We have three sizes of photo on offer:
6x4 inc. card mount
9x6 inc. card mount
12x8 inc. card mount

1 for £5 or 2 for £8 or 3 for £10
1 for £7 or 2 for £11 or 3 for £14
1 for £9 or 2 for £14 or 3 for £18
Payment

We accept bank transfer, PayPal, cheque or cash.
Please let us know what method you would like to use when emailing us and we shall send
you the relevant details with your invoice.
