Access Wireless Representative Agreement
All Identification and Proof of Program Participation document images that are entered into the CGM Enrollment
platform must be clear and legible. If the images are not legible when submitted, a denial or chargeback of
commission will take place. Repeat offenders will have their login terminated.
Changing or altering any of the customer’s information to gain approval is considered fraud and is a felony. Committing
this act will result in the immediate termination of CGM Login and termination from Access Wireless representation.

FCC rules state only one Lifeline discount can be received per household. NO EXCEPTIONS!
Acceptable Identification Documents:
The following are the only forms of identification that can be accepted with an Access Wireless application:
 U.S. Driver’s License (includes District of
 Tribal-Issued ID
Columbia, Puerto Rico and Guam)
 Military of VA ID
 Any State-Issued ID (including state-issued
 U.S. Birth Certificate
medical identification and state-employee
 U.S. Social Security Card
issued ID)
 Certificate of U.S. Citizenship
 U.S. Passport
 U.S. Certificate of Naturalization
 U.S. Permanent Resident Card
 Military Discharge Documentation
 U.S. Permanent Resident Alien Card
Identification MUST name must match the proof of program participation and Access Wireless application.

Acceptable Proof of Program Participation Documents:
Supplemental Nutrition Assistance Program/SNAP/Food Stamps: Complete name match on the ID and Access
Wireless application. No spouse cards accepted. (If working in a state with a State Database, input customer
information exactly as it is typed on the customer’s Proof Document)
Medicaid: Must be an official Medicaid Card. Name on the card must match the name on the applicant’s ID and Access
Wireless application. No spouse cards accepted. Medicare is not accepted.
Supplemental Security Income/SSI: SSI document must be issued to the applicant. The name on the document must
match the name on the applicant’s ID and Access Wireless application. SSI document must say ‘Supplemental Security
Income’. Standard Social Security does not qualify.
Proof supplied for any other program must have appropriate documents as laid out in the Access Wireless Training
Manual. The name on the proof MUST MATCH the name on the identification proof of the applicant.

result in commission chargebacks, termination of login, and possible legal consequences. This document also serves as your
agreement to receive communication from i-wireless via text message and email.

A photocopy of the Rep’s Photo ID must accompany a signed copy of this agreement.
