Outcomes of Sanctioned Encampments for the Homeless
CONSENT FORM
The purpose of this research is to evaluate the outcomes of sanctioned
encampments for the homeless. Responses should be from service providers
that work with sanctioned encampments and its residents. You were selected as
a participant in this study because you were identified as a professional working
with encampment residents. The benefits of participating in this project include
contributing to an expanded perspective of the determinants of a success
encampment for the homeless and contributing to new data that can be reviewed
by homeless services providers.
If you decide to participate in this research, you will be asked to complete the 19
question survey included here. Any information that is obtained in connection
with this study that can reveal your identity will remain confidential. No identifying
words or names will be used on any documents other than the survey.
Participating in this survey is entirely up to you. If you volunteer to participate in
this study, you may request to have your responses withdrawn at any time. You
may also refuse to answer any questions you do not want to answer. The
investigator may withdraw your responses from this research if circumstances
arise which warrant doing so. You are not waiving any legal claims, rights or
remedies because of your participation in this research study.
