Please complete this form in order to have your pet considered for housing at Keen’s Crossing. In
addition to the completed Pet Application & Registration, applicants/residents must provide the
following items for the pet(s): proof that dog(s)/cat(s) have been spayed/neautered, proof of required
vaccinations, and proof of locally required license(s). In conjunction with the completed Pet Application
& Registration items, upon approval of the pet(s), applicant/resident must pay pet deposit of $250 and
sign Pet Policy Addendum.
