For good valuable consideration, including without limitation the acceptance of the undersigned team
member’s entry and or participation in the Cheeky Schoolie Tournament, and also for participating in
activities related to the Cheeky Schoolie Tournament, the undersigned team member, for themselves,
and personal representatives, heirs, guests, invites, employees and team members, hereby (1) RELEASE,
WAIVE, DISCHARGE, AND COVENANT NOT TO SUE the following, which shall be known
collectively as the "Releasees”: Cheeky Fishing, or any of the Cheeky Schoolie Tournament’s sponsors,
contractors or subcontractors, and any of their respective heirs, successors, assigns, officers, directors,
benefactors, employees, agents, affiliates, investors, contractors, or subcontractors.
Furthermore, the undersigned team member (2) AGREE TO ACCEPT FULL RESPONSIBILITY for
their actions and any loss or damage, and all claims or damages therefor, including bodily injury, personal
injury, death, or property damage, which they may suffer or incur during the Cheeky Schoolie
Tournament or that may arise from any activity related to the Cheeky Schoolie Tournament.
The undersigned team member also (3) AGREES TO INDEMNIFY AND HOLD HARMLESS the
"Releasees" from any injury, claim, loss, damage or expense arising from or related in any way to the
Cheeky Schoolie Tournament or any activities related to the Cheeky Schoolie Tournament.
The undersigned team member (4) agrees that this waiver, release, hold harmless, and agreement not
to sue is intended to be as broad and inclusive as permitted by the laws of the Commonwealth of
Massachusetts and that, if any portion is held invalid, it is agreed that the balance shall, notwithstanding,
continue in full legal force and effect.

All INDIVIDUALS MUST COMPLETE A WAIVER. PLEASE EMAIL SIGNED WAIVER TO
GETCHEEKY@CHEEKYFISHING.COM PRIOR TO MONDAY, MAY 16, 2016.
Each participant must complete a waiver by providing his or her name, signature and date. If the waiver
has not been submitted or has been submitted improperly, Cheeky Fishing will notify the participant. The
participant will have a chance to update or correct waiver. The participant will not be able to participate
in the event until it has been corrected. Each participant has the responsibility to return the waiver prior
to the tournament start. If the waiver is not turned in before the start of the tournament, the participant
forfeits their entry fee and they are not considered to be a participant of the Cheeky Schoolie
Tournament.
