Amateur Athletic Release of Liability:

In consideration of being allowed to participate in any way in the European Moo Duk Kwan Tang Soo Do Championships, the undersigned,
the parent or guardian of a minor child participant:
1. Agrees to inspect, prior to participation, the facilities and equipment to be used, and if the participant believes anything is unsafe, he or she should
immediately withdraw from competition and refuse to participate.
2. Acknowledge and fully understand that each participant will be engaging in activities that involve risk of serious injury, including permanent
injury and death, and severe social and economical losses which might result not only from their actions, inaction or negligence, but the
action, inaction or negligence of others, the rules of play or the condition of the premises, or any equipment used. Further that there may
be other risks not known to us or not reasonably foreseeable at this time.
3. Assume all the foregoing risk, and accept personal responsibility for the dangers following such injury, permanent disability, or death.
4. The European Moo Duk Kwan Tang Soo Do Federation is not responsible for damage or loss of the personal equipment used during the tournment
I hereby agree that I am healthy and can participate to the tournament, I will obay the tournamt’s rules that have been explained to me by
my instructor and I will obay the referees and the championship organisers during the period of the tournament.
5. I allow the the orgtanisers to use photos or videos of me.
6. Released, waive, discharge, and convenant not to sue the European Moo Duk Kwan Tang Soo Do Federation, its ‘affiliated clubs, the sports Centre
any and all sponsors, officials, promoters, instructors, or other participants, all of which are hereinafter referred to as “releases”, from any
and all liability to each of the undersigned, his or her heirs and next of kin for any and all claims, demands, losses, damages on account of
injury, including death or damage to property, caused or alleged to be caused in whole or in part by the negligence of the releases or
otherwise.
