X_______________________________________________________ Parent Signature Date ________________ Thanksgiving Coloring Contest Rules Beginning November 1, 2016, and continuing until November 24, 2016, Katiana & Kevin Luttrell will award one of three prizes (see prize list in #7 below) to children between the ages of 4-14 years old who participate in our “Thanksgiving Coloring Contest". Parents will be signed up for our monthly e-Newsletter. 1. Simply color the picture on the front of this flyer. No purchase necessary to enter. 2. Contest is open to children 4-14 years old. 3. Entrants are to color the Thanksgiving scene found here and mail it in with this entry form to: Katiana & Kevin Luttrell RE/MAX College Park Realty 2610 N Los Coyotes Diagonal, Long Beach, CA 90815 4. Entrants may also download a digital copy of the Thanksgiving Coloring Contest from https://goo.gl/vs2TrT , color the Thanksgiving scene, photograph it and this form, and email them to KatianaLuttrell@Remax.net. 5. Limited to one entry per child. Multiple entries will not be considered. 6. The odds of winning depend on the number of entries received. 7. Katiana & Kevin will randomly select three winners of a $50 ToysRUs or Amazon Gift Card, one from each age group: 4-6, 7-9, and 10-14 years old. 8. Deadline for entry: November 24, 2016, at 12:00 PM. Winners will be selected in the afternoon of November 25, 2016. Winners will be notified by phone. 9. Winners will be mentioned on our Facebook page, www.Facebook.com/KatianaHomes 10. Prize is non-transferable. No substitution of prize or exchange for cash. Limit one prize from Katiana & Kevin Luttrell per family or household. 11. Winner consents to the use of his or her name/likeness for publicity purposes without additional compensation. 12.
